    • 2x2 ID Picture

    • Photo of Transaction / Deposit Slip or Proof of Payment

  • Fill-out the registration form. Please make sure all information are true and correct before submitting. After submission, a copy of your response will be sent to your email address.

  • Please expect a payment confirmation email once we validate your payment. Thank you.
